COMPLAINT FORM

NOTICE
Before filing your complaint with the TDLR, first determine if the subject of your complaint is under TDLR’s jurisdiction. TDLR has jurisdiction over Mold Assessors (the person who identifies or evaluates the mold, develops a mold management plan or remediation protocol or analyzes a mold sample) and Mold Remediators (the person or company who removes, cleans, sanitizes, or treats the mold contaminated area). The Texas Department of Licensing and Regulation (TDLR) does not investigate complaints about the presence of mold growth in buildings or homes and does not test for mold. For more information, please refer to the FAQ 's.

In order for the Texas Department of Licensing and Regulation to pursue an investigation of your Mold Assessors and Remediators complaint please provide all documentation and information related to your complaint. If your complaint does not contain enough information for the Department to make a determination that a violation has occurred, your complaint may not be opened for investigation.

When completing section D of this form, important information to support your complaint may include:

  • complete explanation of your complaint
  • name and license number of the company and/or person who performed a mold assessment and/or mold remediation
  • dates mold assessment and mold remediation was performed
  • date you first discovered a problem
  • names, addresses and telephone numbers of any witness you mention in your complaint
  • the approximate size of the area that contained mold contamination
  • The type of structure the mold work was performed on (ex: commercial office, one-family dwelling, apartment, etc.)
  • whether the mold remediation was performed in conjunction with construction
Documentation to support your complaint may include: (Please do not send original documents. All documents you send us will be scanned, electronically saved, and then destroyed.)
  • advertisements (if advertisement is a newspaper or magazine, please include the portion showing the name and date of the newspaper or magazine)
  • business cards
  • proposals
  • contracts
  • receipts or invoices (please include any information that may be printed on the back page)
  • proof of payment (cancelled check, credit card receipt, etc.)
  • photographs – before and after remediation
  • copy of the work analysis prepared by the mold assessor
  • copy of mold remediation work plan prepared by the mold remediator
  • copy of the certificate of mold remediation

Please do not send original documents. All documents you send us will be scanned, electronically saved, and then destroyed. Submitted documentation can only be received via e-mail, fax or regular mail. Attachments cannot be submitted with this link.
