Renew a Nitrous Oxide Permit

Podiatrists must obtain a Nitrous Oxide Permit before using nitrous oxide sedation on a patient for podiatric medical purposes.
Renewal Requirements
Inspection of Equipment
A nitrous oxide permit will not be renewed if a current certificate of inspection of the gas machine is not filed with the department.
Sedation Course
Prior to renewing your permit, you must complete a continuing medical education course in nitrous oxide/oxygen inhalation conscious sedation that includes training in the prevention and management of emergencies in the podiatric medical practice.
CPR Course
You must maintain current certifications in both basic and advanced CPR from either the American Heart Association or the American Red Cross.
Renew Online
To renew your permit, complete an online application form and pay the fee of $25. Required certificates, training, and other documentation can be uploaded.
Certificates are valid for one year from date of issue.
On-Site Inspections
The department may at any time at its own discretion and without prior notification require an on-site office evaluation to determine that all standards regarding nitrous oxide/oxygen inhalation conscious sedation are being met.
Display of Permit
A nitrous oxide permit must be clearly displayed in the physician's office.
Change of Address
Certificate holders must inform the department of any address change or change of hospital setting no later than ten (10) business days after the change is made.
