Student Resources
The Texas Department of Licensing and Regulation (TDLR) has issued an order closing the schools listed below. These closures were based on findings that the school engaged in serious violations of law and rule, including but not limited to fraud, falsification of records, or conduct that compromised the integrity of the licensure and examination processes.
As a result of this closure, TDLR is unable to automatically accept the education hours reported by the school for students. Education hours will not be credited toward licensure unless you provide independent, verifiable evidence demonstrating that you personally completed the required training in accordance with Texas law. This requirement applies to all students of the school, past and present.

Unlicensed Students
What Students Should Do if Impacted
If you attended one of the listed Massage Therapy Schools shown above your options for moving forward with your education to obtain a license are outlined below.
You may either submit documentation to verify the hours you personally completed, or you may choose to voluntarily surrender those hours so you can enroll in another approved school. Students who do not submit the required documentation will not be able to complete their education with another massage therapy school, take the Texas exam, or transfer their hours to another state.
Option 1: Submit Additional Evidence to Verify Education Hours
If you wish to seek credit for education hours completed at the closed school, you must:
- Complete the Affidavit of Self-Certification of Education Hours,
- Complete the Verification of Education Hours Questionnaire, and
- Provide documentation sufficient to independently verify that you personally completed the required training, as indicated in the questionnaire.
TDLR will review all submitted materials and determine, in its discretion, whether they are sufficient to verify the reported hours. Submission of documentation does not guarantee that hours will be approved.
Option 2: Voluntarily Surrender Reported Education Hours
You may choose to voluntarily surrender the education hours reported by the school you attended. If you choose this option, you must;
TDLR will review the submitted document and remove all hours accumulated from the closed school. You may then enroll in another TDLR-approved school to complete the required education.
All required forms and supporting documentation must be submitted to: student.response@tdlr.texas.gov
Licensed Massage Therapist
What This Means for Your License Renewal
If you are a former student who attended one of the listed Massage Therapy Schools shown above and have a current massage therapist or massage instructor license, you will receive notification to provide your documentation at the time of your next license renewal.
The verification of education hours described in this notice is separate from and in addition to any continuing education requirements for license renewal. This verification relates only to the initial education hours credited toward issuance of your license.
Option 1: Submit Evidence to Verify Education Hours
If you wish to keep your license, you must:
- Complete the Affidavit of Self-Certification of Education Hours;
- Complete the Verification of Education Hours (VOEH) Questionnaire; and
- Submit supporting documentation that reasonably verifies completion of the required training, as indicated in the questionnaire.
TDLR will review all submitted materials and determine, in its discretion, whether they are sufficient to proceed with your license renewal process.
Option 2: Voluntarily Surrender License
You may choose to voluntarily surrender your license and your education hours instead of pursuing renewal based on education hours reported by the closed school. If you select this option, your license will be canceled and no longer valid, and any pending license renewal application will be considered withdrawn.
If you later seek licensure, you will be required to meet all statutory and rule requirements in effect at the time of application, including completion of any required education through a licensed school.
To proceed with this option, you must:
- Complete the Voluntary License Surrender Form; and
- Complete the Affidavit of Voluntary Surrender of Education Hours.
Acceptable Supporting Documentation
- Official school records (these alone cannot support your verification of education hours, but may be used to help clarify other supporting documents);
- Statements from instructors or supervisors with direct knowledge of your training;
- Records demonstrating travel to and from the school for in-person instruction;
- Time-stamped attendance or system logs; or
- Other reliable, contemporaneous records clearly tied to your education.
All required forms and supporting documentation must be submitted to: student.response@tdlr.texas.gov
Student Refunds
Pursuant to Texas Administrative Code §117.66, the school is responsible for issuing refunds to eligible students. Please refer to your contract with the school regarding the school’s refund policy.
If eligible, students can move forward with the refund process by considering the following options:
- Contact the owners of the school directly to request a refund.
- Consult an attorney to seek legal advice regarding your options for recovering a refund from the school.
