Application and Fee
To apply for a new provider license with an online, in-person, or both online and in-person endorsement,please submit a completed Court-Ordered Program Application (PDF) with the application fee of $300.
Apply Online
Licenses are valid for a period of two years from the date issued.
You cannot enroll students or schedule or begin any classes until your license application has been approved. However, you may advertise as a prospective school that is opening soon.

Endorsement Types
The court-ordered program provider license has four possible endorsements for the program that will be taught:
- Alcohol Education Program for Minors (AEPM)
- Drug Offense Education Program (DOEP)
- DWI Education (DWIE)
- DWI Intervention (DWII)
When you apply for a Court-Ordered program license, the first endorsement is included with your license application fee.
Subsequent endorsements require an additional fee of $280/each.

Instructors
All program providers must have a designated instructor endorsed to teach the same endorsement(s) as the provider.
You must notify the department in writing within 30 days of any change to the designated instructor.
