Apply for a New License

PEO License (Full License)

To apply for a PEO license (full license), an applicant must submit:

  1. a completed PEO License Application Form - PEO-LIC-001;
  2. a Certificate of Insurance for workers’ compensation, if the applicant will offer or sponsor workers’ compensation;
  3. a completed PEO Personal Information Form - PEO-LIC-003 for each controlling person;
  4. the fingerprint submissions for all controlling persons;
  5. the Audited Financial Statements;
  6. the Corporate Information Form - PEO-LIC-005, if applicable;
  7. any required attachments specified in the application instructions; and
  8. the required fees.

The application and fees will need to be mailed to the Texas Department of Licensing and Regulation, P.O. Box 12157, Austin, Texas 78711.

The fees are as follows:

You must wait until you receive the license issued to you before you can start operating in Texas.

If there is any change to the initial license application after submission, the applicant must update the information provided to the Department within 45 days after any change to the information.

PEO Limited License

To apply for a PEO limited license, an applicant must submit:

  1. a completed PEO Limited License Application Form - PEO-LIC-017;
  2. a completed PEO Personal Information Form - PEO-LIC-003 for each controlling person;
  3. a Certificate of Insurance for workers’ compensation, if the applicant will offer or sponsor workers’ compensation;
  4. the Audited Financial Statements
  5. a Corporate Information Form - PEO-LIC-005, if applicable;
  6. any required attachments specified in the application instructions; and
  7. the required fees.
