Renew a Hyperbaric Oxygen Permit

Hyperbaric oxygen permits a re valid for one year and must be renewed annually. You should receive a reminder email or postcard from TDLR approximately 60 days before your license expires. Please ensure that your contact information on record is current and up-to-date to receive these notifications.

You may renew online and upload required certificates, training or other required documentation. There is a nonrefundable fee of $25.

Certificates are valid for one year from date of issue.

Display of Permit

A hyperbaric oxygen certificate must be clearly displayed in the office alongside the original license.

Change of Address

Certificate holders must inform the department of any address change or change of hospital setting no later than ten (10) business days after the change is made.
