Effective June 9, 2025, the department will no longer accept annual reports by fax, email, or mail
If you offer more than one course, you must submit a separate annual report for each course.
If you have already submitted the annual report for this year, you must resubmit the report using the Online Licensing Services System.
To submit an Annual Report,
- log into the Online Licensing Services System;
- select “Court Ordered Provider Submit Annual Report”;
- follow the reporting instructions, and.
- click “Submit” to finish.
