Apply for a Temporary Practitioner License

If you have practiced orthotics or prosthetics for at least the past 5 years, and you have been licensed in another state, you may be eligible to receive a temporary practitioner license, which allows you to practice in Texas while your application for a practitioner license is still under review by the department.

Temporary licenses are valid for 1 year from the date of issue. If your practitioner application is not approved, the temporary license will no longer be valid.

Eligibility

To be eligible for a temporary license, you must first apply for a practitioner license in Texas.

The state where your license was issued must have licensing requirements that are equal to or exceed the Texas requirements.

Application and Fees

To apply for a temporary license, submit a completed Orthotists and Prosthetists License Application Form (PDF) along with the appropriate fee, specified below. When completing the application form, specify your "Licensure Pathway" as "Temporary".

Online applications are not accepted for temporary licenses.
