Name Change for Establishments and Mini-Establishments

Application and Fee
To change the name of your establishment or mini-establishment, submit a completed Notice of Change and Duplicate License Request (PDF) form.
There is a non-refundable application fee of $25 to process the change.
You may either submit the completed form via secure webform, or you can mail all documents to the address listed on the form.
During License Renewal
If you are within 90 days of your license expiration, then you can process the name change as part of the renewal and avoid paying the $25 fee. Please send your request for a name change with via our secure webform and include the following information:
- The current establishment name
- The new establishment name
- The license number of the establishment
- The owner of the establishment's name
You may also choose to update your mailing address, phone number, and email address at the same time by completing those fields in the form.
You can also update your contact info online if you prefer.
Processing Time
Name changes can take multiple weeks to receive and process.
If you need to renew your license concurrently with a name change, please complete the name-change process before renewing your license.
